29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) Job site, near 1150 Smallman Street, Pittsburgh, PA: On or about December 15, 2017, during excavation activities, the employer did not ensure that an appropriate inspection was made by a competent person in an excavation determined to be at least approximately 8 feet deep. Deficiencies noted during the inspection included, but were not limited to, insufficient structural supports for shoring in some areas of the excavation and in other areas no shoring was used exposing employees to the hazard of excavation collapses along vertical/near vertical wall faces.
