(b) Funeral Home - An embalmer, exposed to formaldehyde during embalming operations at an 8 hour time-weighted average level of 1.7 ppm, was not provided a medical evaluation prior to being required to use the 3M 6900 full-face model tight-fitting respirator provided to protect against exposure to Triton 28, a formaldehyde product used in embalming operations, on or about January 10, 2018.
