Periodic inspections of the energy control procedures had not been performed since 2013.    In accordance with the requirements of 29 CFR 1903.19(c), abatement certification only is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a) Cold Nut Forming - On or about December 5, 2017, the employer did not ensure that lockout/tagout was performed and energy isolating devices were affixed to each energy source when machine operators performed a transfer cylinder change on the MM16- Nitrov Cold Former machine.  An employee suffered a finger amputation on December 5, 2017 when performing a cylinder transfer change.      In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.
