29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  TUMBLING  On, or about December 19, 2017, the employer failed to re- assess the workplace to determine if hazards were present, which necessitated the use of personal protective equipment when  a new chemical, Trowalpur V,  which contained aluminum sulphate was introduced to the process, and required the use of chemical gloves.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  TUMBLING  On, or about December 19, 2017, employees were not trained on the hazards associated with TROWAL PUR V, a replacement flocculant used in the tumbling centrifuge system, which contained aluminum sulphate, which may cause contact dermatitis.
