29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about December 18, 2017, employee operating an JLT aerial lift at a construction site was performing framing work and did not have a lanyard attached to the basket. This condition exposed employee to falls exceeding 10 feet to the ground level.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   On or about December 18, 2017, the employer did not ensure that employees were provided with fall protection training necessary for hazard recognition for safe work practice. Employee was operating the airlift without fall protection equipment. This condition exposed employees to falls exceeding 10 feet.

29 CFR 1904.31(a): Basic requirement. You must record on the OSHA 300 Log the recordable injuries and illnesses of all employees on your payroll. You also must record the recordable injuries and illnesses that occur to employees who are not on your payroll if you supervise these employees on a day-to-day basis.  On or about December 18, 2017, employer did not keep records of OSHA 300, 300A, 301 forms.
