29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: Northwest side of the town homes being re-constructed: On or about December 20, 2017, employees without fall protection were exposed to fall hazards greater than 10 feet to the ground below, while installing roof truss utilizing two metal frame scaffold platform that was incomplete.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Northeast side of the townhomes being re-constructed: On or about December 20, 2017, employees with no fall protection were exposed to fall hazards greater than 6 feet while working from the roof installing the roof truss.
