29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level   a) On or about December 20, 2017; Basement area; Employees were using step-ladders on an uneven concrete floor exposing them to a potential fall hazard if the ladders become displaced.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:   a) On or about December 20, 2017; Basement area; the ladders used in the process were not inspected by a competent person ensuring the equipment was in good repair or removed if unsafe.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  a) On or about December 20, 2017; Basement area; The employer had not provided training by a competent person on the hazards associated with the use of  portable ladders, inspecting the equipment, removal procedures, proper use and determining the weight restrictions of the ladder prior to use.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
