29 CFR 1910.28(b)(3)(i): Each employee was not protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level: a) Sintergy, Inc., production area. On or about December 20, 2017, a hole that was 80 inches by 74 inches wide, was in the floor, near the powder metal presses. Employees were exposed to a fall of 7.5 feet into the hole.
