29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION/CONDITION) (DESCRIBE HAZARD) Note: The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator. a. Abrasive blasting enclosure - The employer did not provide respirator medical evaluations when employee(s) were required to use a RPB Nova 2000 supplied air respirator during abrasive blasting operations. Violation discovered on or about December 21, 2017. Hammer Haag Steel, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 1223513 citation number 1, item number 1-C and was affirmed as a final order on 10/16/2017, with respect to a workplace located at 12707 US Hwy 19 North, Clearwater, FL 33764.

29 CFR 1910.134(i)(1)(ii): Breathing air did not meet requirements for Grade D breathing air as described in the ANSI/Compressed Gas Association Commodity Specification G-7.1-1989: a. At the site- The employer did not test the breathing air to ensure the compressed breathing air supplied by the RPB Nova 2000 supplied air respirator system used with the Ingersol Rand air compressor system met at least Grade D breathing air quality. Violation discovered on or about December 21, 2017. Hammer Haag Steel, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.134(i)(1)(ii), which was contained in OSHA inspection number 1223513 citation number 1, item number 2-A and was affirmed as a final order on 10/16/2017, with respect to a workplace located at 12707 US Hwy 19 North, Clearwater, FL 33764.
