29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) On or about December 20, 2017, the employer did not ensure that a Lincoln Arc Welder used to weld parts in the main shop was free from strain where the outer sheath of the main power cord was pulled from the plug, exposing the wires inside. (b) On or about December 20, 2017, the employer did not ensure that a Millermatic Arc Welder used to weld parts in the main shop was free from strain where the outer sheath of the main power cord was pulled away from the plug, exposing the wires inside. Abatement note: Abatement Certification is required for this item (See Certification of Corrective Action Worksheet).

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: (a) On or about December 20, 2017, the employer did not ensure that a plasma cutter was removed from service where several sections of the main cord and the cord supplying the cutter had damage, including exposed bare wire.
