29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) 344 North 21st Street, Lebanon Pennsylvania - Employees were working on a roof without fall protection exposed to a 10 foot fall hazard.  The employer did not ensure that employees on the roof wore fall protection, on or about December 21, 2017.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) 344 North 21st Street, Lebanon Pennsylvania, front of the house - An employee used a ladder to access the roof.  The employer did not ensure that the ladder was extended 3 feet above the roof , on or about December 21, 2017.      (b) 344 North 21st Street, Lebanon Pennsylvania, back of the house - An employee used a ladder to access the roof.  The employer did not ensure that the ladder was extended 3 feet above the roof , on or about December 21, 2017.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement
