29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 21 December 2017, and at times prior thereto, two workers performing window renovations and installations were doing so from multi-tiered scaffolds without the use of hard hats, exposing them to the hazard of being struck by falling tools, materials, or debris.

29 CFR 1926.451(h)(2)(ii): A toe-board was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below.  On or about 21 December 2017, and at times prior thereto, two workers performing window renovations and installations did so from two tubular-welded frame scaffold structures which did not have toe-boards in place, exposing the workers to the hazard of being struck by falling tools, materials, or debris.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  On or about 21 December 2017, and at times prior thereto, two workers were performing work on a tubular-welded frame scaffold at various levels in which those levels were not fully planked, exposing the workers to a fall hazard anywhere between approximately 6 feet 6 inches up to approximately 84 feet 6 inches in height.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units.  On or about 21 December 2017, and at times prior thereto, two workers used a tubular-welded frame scaffold to perform work on exterior windows in which two of the legs were supported at the foot with pieces of wood stacked up to make the scaffold level, exposing the workers to a fall hazard from approximately 6 feet 6 inches up to approximately 84 feet 6 inches. During the erection of the scaffold, a 6-man crew built the structure.

On or about 21 December 2017, and at times prior thereto, two tubular-welded frame scaffold structures did not have mid-rails in place while workers performed work from the scaffolds, exposing the workers to a fall hazard from approximately 6 feet 6 inches up to approximately 84 feet 6 inches.
