29 CFR 1918.51(b): All components of cargo handling gear, including tent gantlines and associated rigging, were not inspected by the employer or a designated person before each use and at appropriate intervals during use. a) Barge Terrance: No proper and effective inspections were done at the site for hazards found on the gantry crane "Terrance" Clyde gantry crane model number 28 HD 120, Serial number CW 3911. The crane was found to have damaged walkways and platforms, covers missing off electrical receptacle outlets and machinery, a rusted out cab floor, over abundance of grease, oil, and grime covering most metal components on the super structure, and alarms which were non-functional, that should have been operational to worn employees when the crane was moving.

29 CFR 1918.55(a): Cranes with a visible or known defect that affects safe operation were used. a) Barge Terrance: The gantry crane "Terrance" Clyde gantry crane model number 28 HD 120, Serial number CW 3911. The crane was being used with damaged walkways and platforms, covers missing off electrical receptacle outlets and machinery, a rusted out cab floor, over abundance of grease, oil, and grime covering most metal components on the super structure, and alarms which were non-functional, that should have been operational to worn employees when the crane was moving.

29 CFR 1918.55(c)(1): Accessible areas within the swing radius of the body of a revolving crane or within the travel of a shipboard gantry crane were not physically guarded to prevent an employee from being caught between the body of the crane and any fixed structure, or between parts of the crane. NOTE: Verbal warnings to employees to avoid the dangerous area do not meet this requirement. a) Barge Terrance: When gaining access to the Terrence gantry crane there were several areas where someone on the outer superstructure could be struck by the crane structure (including platforms and ladders). Employees could be crushed between the superstructure and the crane body or they could be knocked off the platform. When CSHO cleared the area to allow the crane to rotate to have two access ladders line up, there were no warning sounds from alarms and no physical guards to prevent employees from entering the danger zone.
