29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that that electrical equipment was in a non-energized state: Or in the alternative: 29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: (a) On or about December 17, 2017 for the employees exposed to electrical shock hazards who did not verify that the MDP-3 panel was in a non-electrical energized state prior to performing maintenance activities at the Integrity Windows and Doors plant located on 1616 43rd Street North Fargo, ND.
