Location: Jobsite.  Employees were exposed to struck-by hazards from flying debris, while operating a nail gun, and were not protected by safety glasses.

Location: Jobsite.  Employees were exposed to a 16 foot fall hazard while doing framing work, where fall protection had not been used or provided.

Location: Jobsite.  Employees were exposed to fall hazards greater than 16 feet.  Employer did not provide fall hazard training for example,  hazards recognition and avoidance and how to wear and use personal fall arrest systems.
