Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.    (a)  On or about December 22, 2017, stacks of bundled and palletized paper were stacked out of alignment and had tipped over created struck-by hazards to forklift operators removing the material.    (b)  On or about December 22, 2017, pallet racking located in the south east corner of the building had damage to the support columns and beams that were out of alignment and missing cross members creating struck-by hazards if the racking would fail under load.
