The employer did not institute a training program and ensure employee participation in the program. a) At the site, on or about and at times prior to December 22, 2017, the employer did not ensure employees installing metal decking on a steel building, that measured between sixteen and eighteen feet in height, were trained on fall hazard recognition and the use of fall protection, exposing employees to fall hazards.
