29 CFR 1926.451(c)(1)(ii): Guys, ties or braces were not installed at the closest horizontal member for scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) and repeated vertically at locations of horizontal members every 20 feet or less thereafter for scaffolds 3 feet wide or less, and every 26 feet or less thereafter for scaffolds greater than 3 feet wide: a) On December 19, 2017, employees were exposed to fall hazards of approximately 22.7 feet while working on a frame scaffolding which was not restrained from tipping by any means.
