29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them:    (a) On or about November 28, 2017, Aleut Facilities Support Services, LLC, as the creating contractor, did not ensure that effective frequent and regular inspections of the jobsite were conducted by a competent person so as to ensure that employees of Pipe Line Specialists, Inc. were protected from jobsite hazards, including but not limited to,  an isolated water line that was pressurized with air that ruptured during excavation operations.
