29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Harris Rebar Salt Lake, Inc. at 5500 Colorado Blvd. Denver, CO 80022: On or before December 18, 2017 the employer did not ensure that employees were protected from injury, in that an employee operating the CRS MOD 35 Radius Machine sustained a serious hand injury after coming in contact with the ingoing rotating parts and the point of operation of the machine. This condition exposed employees to amputation hazards.
