OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that:    a) Employee(s) responsible for collecting and chipping limbs / branches on the ground, during trimming phase of tree removal along Old Nassau Rd in Rossmoor community in Monroe, NJ were exposed to hazard of being struck by falling tree limb(s).      Condition(s) were noted on or about 12/15/2017.    NOTE: Abatement documentation is required for this item in accordance with 1903.19

29 CFR 1904.29(b)(1): 29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:    a) Description(s) of recordable injuries on the 2017 OSHA 300 Log (Column F) did not include information on the object or substance that caused injury, and the only info entered for location where the event occurred (Column E) was the city.  Among the recordable injuries with object / substance omitted from the description in Column f was a 12/15/2017 incident recorded as "leg & knee injury" caused by a cut / dropped tree branch that struck a worker and knocked them to the ground.    Condition(s) were noted on or about 6/04/2018.
