29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.    On or about December 27, 2017, three carpenter employees installing plywood sheeting on the roof were exposed to 12 feet to 19 feet fall hazards from the edges of the 7:12 pitch roof in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.    On or about December 27, 2017, at the front of the house, the roof access ladder did not extend at least 3 feet above the edge of the roof, exposing three carpentry employees to a 10 feet fall hazard from the eave.
