29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: Note: If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: On or about December 12, 2017, and at times prior thereto, the employer added a job-built wooden trash box attachment to the JLG rough terrain forklift without the manufacturer's written approval, exposing workers to the hazard of being struck by the trash box or debris.
