29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, paragraphs 1910.147(c)(4)(ii)(A) - 1910.147(c)(4)(ii)(D): The energy control procedures did not outline: 1. A specific statement of the intended use of the procedure. 2. Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. 3. Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and 4. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. An example includes the 280 Volt Power Supply UD3A2 Removal Procedure in the NWS EHB 6-511: 5.3.5 280 VOLT POWER SUPPLY UD3A2. The following procedures describe how to remove and install 280 Volt Power Supply A2. Related alignment and performance test procedures are also referenced. See Figure FO11-13, Sheet 2, view A and locate 280 Volt Power Supply A2. Two technicians are required for this procedure. 5.3.5.1 Equipment and Tools Required. 1. Screwdriver set, flat-tip 5.3.5.2 Initial Conditions/Preliminary Setup. WARNING Hazardous voltages are present within the transmitter cabinet. Take all standard precautions against electric shock. Failure to comply may cause serious injury or DEATH. 1. Perform paragraph 3.4.1.2, steps 1 through 4 to power down the transmitter, lock HIGH VOLTAGE POWER CB1 circuit breaker, and remove the key. WARNING HIGH VOLTAGE Filter Capacitor Bank A9 can hold a lethal charge long after transmitter power has been removed. Discharge Filter Capacitor Bank A9 after transmitter power is off and prior to transmitter maintenance by opening the right bay inner door while monitoring the charge on VOLTAGE/CURRENT Meter A1M4, at position 6, +280 VDC power supply. Opening the right bay inner door releases the filter capacitor bank discharge plunger. Failure to comply may cause serious injury or DEATH. 2. Use interlock key to unlock and open right bay inner door to discharge residual high voltage from Filter Capacitor Bank A9. 3. Use interlock key to unlock and open left bay inner door. 5.3.5.3 280 Volt Power Supply UD3A2 Removal Procedure. See Figure FO11-13, Sheet 2 and Figure FO11-13, Sheet 5, detail G and proceed as follows: 1. Disconnect cable from A2J1 on front of 280 Volt Power Supply A2.
