29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On December 19, 2017 at the worksite located at 1784 Hwy 1 N in Wynne, Arkansas, an employee received an injury resulting in an inpatient hospitalization. The employer failed to report the inpatient hospitalization to OSHA within twenty-four (24) hours as required.
