29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: a) An employee was exposed to a caught-in/struck-by hazard when he attempted to drive a scissor lift, without the supervision and direction of a competent person, through a double door to access the outside, in that the opening was 85 inches high and the top rail of the scissor lift was 78 3/4 inches high, leaving only a 6 1/4 inch clearance between the door frame and the top rail of the scissor lift.
