29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: (a) Revolution Industrial LLC, at 2001 Blake Street, Denver, CO: On and preceding 1/2/18, Revolution Industrial LLC did not ensure that a horizontal lifeline was designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two. Employees performed structural steel work in preparation of installation of a scoreboard. Work took place on multiple levels above the concrete walkway at the base of the steel structure. A 3/8 inch wire rope was strung across each level at the exposed edge, spanning approximately 119 feet. The wire rope was utilized as a horizontal lifeline. The employer did not ensure that a qualified person supervised the design and installation of the 3/8 inch wire rope being utilized as a horizontal lifeline. This condition exposed employees to approximate 15.5 foot, 24 foot, 32.5 foot, 41 foot, 49.5 foot, and 58.5 foot fall hazards.
