29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system: (a) RK Mechanical Inc, dba RK Electrical, at 2001 Blake Street, Denver, CO: On and preceding 1/2/18, RK Mechanical Inc did not ensure that each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system. Employees performed electrical work in preparation of installation of a scoreboard. Work took place on multiple levels, including the first level, second level, and sixth level, above the concrete walkway at the base of the steel structure. A 3/8 inch wire rope was strung across each level at the exposed edge, spanning approximately 119 feet. Four foot deep vertical structural steel divided each level at the exposed edge into three zones. Employees were provided DBI/SALA full body harnesses with DBI/SALA Nano-Lok single leg self-retracting lifelines (SRL). The SRL was connected to the 3/8 inch wire rope. Employees disconnected from the wire rope when traveling between zones, passing the vertical structure steel prior to re-connecting to the wire rope on the other side of the vertical structural steel. The employer did not ensure that employees were protected from a fall when traveling between zones. This condition exposed employees to approximate 15.5 foot, 24 foot, and 58.5 foot fall hazards.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: (a) RK Mechanical Inc, dba RK Electrical, at 2001 Blake Street, Denver, CO: On and preceding 1/2/18, RK Mechanical Inc did not ensure that a horizontal lifeline was designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two. Employee performed electrical work in preparation of installation of a scoreboard. Work took place on multiple levels, including the first level, second level, and sixth level, above the concrete walkway at the base of the steel structure. A 3/8 inch wire rope was strung across each level at the exposed edge, spanning approximately 119 feet. Employee was provided DBI/SALA full body harnesses with DBI/SALA Nano-Lok single leg self-retracting lifelines (SRL). The SRL was connected to the 3/8 inch wire rope. The employer did not ensure that a qualified person supervised the use of the 3/8 inch wire rope being utilized as a horizontal lifeline. Employees performed electrical work in preparation of installation of a scoreboard. Work took place on multiple levels, including the first level, second level, and sixth level, above the concrete walkway at the base of the steel structure. A 3/8 inch wire rope was strung across each level at the exposed edge, spanning approximately 119 feet. Employee was provided DBI/SALA full body harnesses with DBI/SALA Nano-Lok single leg self-retracting lifelines (SRL). The SRL was connected to the 3/8 inch wire rope. The employer had provided fall protection training focused upon PFAS use with a single anchor point. The employer had not provided retraining with regards to use of a horizontal lifeline. This condition exposed employee to approximate 15.5 foot, 24 foot, and 58.5 foot fall hazards.
