29 CFR 1910.219(i)(2): Revolving surfaces of shaft coupling(s) were not covered by a safety sleeve:     (a) Omni Baking Company Loading area, Freightliner K cab tractor:  On or about January 2, 2018 a Foodliner Inc. employee was exposed to the high speed rotating, unguarded coupling/universal joint of a power take off shaft used to drive a Gardner Denver Cycloblower.  The employee's head  made contact with the coupling/universal joint resulting in serious injury.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
