29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  On or about January 3, 2018, at the 600 block of Commercial Street in Neenah jobsite; cave-in protection was not utilized while an employee was in a trench greater than five feet deep exposing employees to struck-by hazards.
