29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:    a.  On or about January 3, 2018, in the facility, the employer had not performed a personal protective equipment assessment which had identified and assessed all the hazards necessitating the use of personal protective equipment.  Employees exposure which necessitated personal protective equipment included welding flash, grinding debris and hazardous paints.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment. Each such employee shall be trained to know at least the following:  a.  On or about January 3, 2018, in the facility, employees were not trained to know at least the following:    1.  What PPE is necessary. 2.  When PPE is necessary. 3.  How to properly don, doff, adjust and wear the PPE. 4.  The limitations of PPE. 5.  The proper care, maintenance, useful life and disposal of the PPE.  Employee exposure included but was not limited to  welding flash, grinding debris, styrene and xylene.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use    a.  On or about January 3, 2018, in the facility, employees were required to wear tight fitting respiratory protection when painting and welding.  Employees were exposed to hazards associated with welding fumes and paint vapors when the employer failed to develop and implement a respirator program.

29 CFR 1910.305(a)(2)(x): Flexible cords were not protected from accidental damage caused by sharp corners, doorways and pinch points.:    a.  On or about January 3, 2018, in the new paint area, employees were exposed to electrical hazards from flexible wiring used to power a portable lamp.  The flexible electrical cord was used through a metal doorway with pinch points and was not protected from damage.
