29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.    On or about, January 04, 2018, employees were working on the roof of a residential home without using any form of fall protection. Employees were exposed to fall hazards of more than six feet to the ground below.

29 CFR 1926.503(c)(3): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:      On or about January 04, 2018, employees were working on the roof of an residential home  without having adequate understanding and skill in using fall protection equipment. This condition exposed the employee to fall hazards greater than six feet.
