29 CFR 1926.502(i)(2): All other covers were not capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. A) Ramsey Development LLC on or about December 01, 2017, at the address of 370 N. Eastown Road, Lima, OH: The employer directed that a styrofoam sheet be used to cover 4 ft. x 4 ft. mechanical equipment access holes used for placement of HVAC systems. A subcontractor employee stepped on a cover which failed, resulting in the employee falling 10 feet to the level below.
