29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: Ongweoweh SCS St. Louis, LLC, East St. Louis, Illinois: On or about December 10, 2017, the employer did not report the following workplace injury to the Occupational Safety and Health Administration: On or about December 9, 2017, a reportable injury occured when an employee received a surgical finger tip amputation due to smashed finger from a Smart Products Manual Trim Saw.
