29 CFR 1904.32(a)(1): The OSHA 300 Log was not reviewed to verify that the entries are complete and accurate. Deficiencies were not corrected.     On or about January 5, 2018, at the jobsite located 111 Boulder Industrial Drive, Bridgeton, Missouri, 63044 the employer did not verify the entries in the OSHA 300 Logs were complete and accurate.  Where deficiencies were evident they were not corrected.

29 CFR 1904.32(b)(1): The employer did not review the OSHA 300 Log entries as extensively as necessary to make sure the entries were complete and correct:     On or about January 5, 2018, at the jobsite located 111 Boulder Industrial Drive, Bridgeton, Missouri, 63044 the employer did not adequately review the OSHA 300 Log entries for completeness and correctness. Some logs were left with columns that had not been totally as required.

29 CFR 1910.134 App D: Appendix D to Sec. 1910.134 (Mandatory) Information for Employees Using Respirators When Not Required Under the Standard (Construction Reference 1926.103):   On or about January 5, 2018, at the jobsite located 111 Boulder Industrial Drive, Bridgeton, Missouri, 63044 where the employer provided N95 Sanding and Fiberglass Respirators (3M Model # 8200HB2) for voluntary use; the following requirements were not met:     1. Employers did not determine that the masks themselves do not pose a hazard to workers; and     2. Employers did not provide the information found in Appendix D to 1910.134 of OSHA's Respiratory Protection Standard to workers on a one-time basis. This appendix provides important information the employee needs to know about wearing dust masks.
