29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    An employee was replacing the thin wires on the tortilla machine and failed to de-energize and control the hazardous energy, exposing the employee to an amputation hazard.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.    A work related inpatient hospitalization and amputation occurred on December 11, 2017 and the employer failed to notify OSHA.
