29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a)  On or about January 8, 2018, an employee was exposed to overhead falling object hazards while cleaning debris on the ground below roofing operations without wearing a hard hat. 29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) On or about January 10, 2018, and at times prior, Paramount Enterprises LLC dba Paramount Roofing and Gutters, as controlling employer, did not ensure that workers of Castle Roofing LLC, subcontractor, and Jose Perez Roofing, subcontractor, were safe while performing roofing activities with pneumatic nail guns, and working adjacent to the pneumatic nail guns, in that they were not wearing safety glasses and were exposed to eye injuries from kickback, misfire, and flying debris. 29 CFR 1926.502(d)(6)(i): Snap hooks were connected directly to webbing, rope or wire rope:  (a)  On or about January 10, 2018, and at times prior, Paramount Enterprises LLC dba Paramount Roofing and Gutters, as controlling employer, did not ensure employees of Jose Perez Roofing, subcontractor, and Castle Roofing, subcontractor, were protected from falls when using a vertical lifeline that wrapped around metal lattice on apartment buildings and the snap hook was attached to the rope.
