29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): This violation was most recently observed on or about December 7, 2017, and at times prior thereto at United Initiators, Inc. facility located at or near 5614 Phillips Road, Helena AR. The employer failed to develop and implement al hazard communication information program for employees who are exposed to hazardous chemicals, including but not limited to Dibenzoyl Peroxide used by the United Initiators, Inc. employees.

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and -2 on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area. This violation was most recently observed on or about December 7, 2017, and at times prior thereto at United Initiators, Inc. facility located at or near 334 Phillips 311 road, Helena AR. The employer failed to provide training to their employees to explain Hazard Communication Standard requirements relating to their exposure to benzoyl peroxide.
