29 CFR 1910.22(a)(3): (3) Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a) On December 30, 2017 while securing a load for shipment the working surface of the loading truck was not free from hazards, in that snow and/or ice was not removed from the surface exposing employees to fall hazards.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) The employer did not report an injury within twenty-four (24) hours of an employee being injured and hospitalized, in that an injury occurred on December 30, 2017 and the reported injury was on Jan 3, 2018.
