29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     (a). Worksite located 531 Grand St., Kansas City, Missouri- Employees were applying mortar to bricks without the use of any means of eye protection.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:    (a). Worksite located at 531 Grand St., Kansas City, Missouri- Employees were exposed to fall hazards and had not been trained on the proper use of fall protection equipment. Employees were using inadequate anchors and improper equipment to prevent falls.
