On February 8, 2018 employees were exposed to struck-by hazards, in that the employer did not ensure a lock was affixed to the power source of the 100 T Stamtec, Company Number 17 press during die changes.

29 CFR 1910.217(b)(13): Control reliability. When required by paragraph (c)(5) of this section, the control system shall be constructed so that a failure within the system does not prevent the normal stopping action from being applied to the press when required, but does prevent initiation of a successive stroke until the failure is corrected. The failure shall be detectable by a simple test, or indicated by the control system. This requirement does not apply to those elements of the control system which have no effect on the protection against point of operation injuries.    On January 3, 2018 employees were exposed to a struck-by hazard, in that the Bliss part revolution mechanical power press, Company Number 6, did not have a control system that was constructed so that a failure within the system does not prevent normal stopping of the press when required or prevent initiation of a successive stroke.

29 CFR 1910.217(b)(14)(i): Be so constructed as to automatically prevent the activation of a successive stroke if the stopping time or braking distance deteriorates to a point where the safety distance being utilized does not meet the requirements set forth in paragraph (c)(3)(iii)(e) or (c)(3)(vii)(c) of this section. The brake monitor used with the Type B gate or movable barrier device shall be installed in a manner to detect slide top-stop overrun beyond the normal limit reasonably established by the employer.    On January 3, 2018 employees were exposed to a struck-by hazard on the Bliss 60 T press, in that the press was not constructed with a  brake monitoring system that would prevent the activation of a successive stroke if the stopping time or braking distance deteriorates.

On January 3, 2018 employees were exposed to a struck-by hazard, in that the employer did not ensure that only the supervisor or a safety engineer were the only ones capable of relocating two hand controls, in that the controls were not secured in place on three presses (Stamtec G2-110 Company Number 17, 125 T Johnson Company Number 1, and the Stamtec G2-200 Company Number 16).

29 CFR 1910.217(e)(1)(i): It shall be the responsibility of the employer to establish and follow a program of periodic and regular inspections of his power presses to ensure that all their parts, auxiliary equipment, and safeguards are in a safe operating condition and adjustment. The employer shall maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected.    On January 3, 2018 employees were exposed to struck-by hazards in that there were 12 presses (60 T Bliss #6, 110 T Danly #110, 100 T Stamtec #17, 60 T Bliss #4, 105 T Verson #27, 80 T Stamtec #18, 125 T Johnson #1, 200 T Stamtec #16, 27 T Johnson #22, 22 T Bliss #11, 250 T Verson #38, 250 T Verson #34) in use that were not being periodically inspected to ensure all their parts, equipment and safeguards were in safe operating condition.

29 CFR 1910.217(e)(3): Training of maintenance personnel. It shall be the responsibility of the employer to insure the original and continuing competence of personnel caring for, inspecting, and maintaining power presses.    On January 3, 2018 the employer did not insure that the personnel caring for, inspecting and maintaining power presses were competent in doing so, in that annual inspections were not completed on all presses.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.    On January 11, 2018 employees were exposed to a shock hazard, in that there was an unused opening on a box located on the 200 T Stamtec .
