29 CFR 1910.1048(h)(1)(iii): When a face shield was worn, chemical safety goggles were not required where there was a danger of formaldehyde reaching the area of the eye:  (a) Employees working in the embalming room were given a face shield that was worn but required chemical safety goggles were not provided and being used with the face shields where there was a danger of formaldehyde reaching the area of the eye during the embalming process.  The embalming chemicals contain formaldehyde which can cause serious eye damage. Chemical Safety Data Sheets recommend use of goggles.

29 CFR 1910.1048(i)(3): When there was a possibility that an employee's eyes could be splashed with solutions containing 0.1 percent or greater formaldehyde, the employer did not provide acceptable eyewash facilities within the immediate work area for emergency use:    (a) Emergency eye wash was not available to employees. The eyewash in the embalming room was broken. The embalming chemicals contain formaldehyde which can cause serious eye damage. Employees working in the embalming room wore a face shield but no chemical goggles. Employees are exposed to splash hazards to the eyes during the embalming process.
