Popeye Construction, LLC, dba B&M Construction, project site located at 700 S. 8th St. Sheboygan, WI: On and about January 11, 2018, employees were exposed to falls while working on elevated surfaces when:   A) no fall protection was used by an employee walking on a bundle of floor trusses placed on top of walls;  B) employees laying OSB decking were able to fall greater than 6' while working laterally from their anchors at a greater distance than to the nearest edge;  C) employees had not adjusted their harnesses to ensure the dorsal D-rings stayed between their shoulder blades.
