29 CFR 1910.25(b)(1): The employer did not ensure handrails, stair rail systems, and guardrail systems were provided in accordance with 1910.28:   A) Small Dryer Room Area, on or about January 10, 2018, employees used stairs in the small dryer room that had an opening greater than 19 inches between the step and the top rail of the stair rail  system, exposing employees to a fall hazard.   B) Outside small dryer room, on or about January 10, 2018, employees used two sets of  stairs that had an opening greater than 19 inches between the step and the top rail of the stair rail system, exposing employees to a fall hazard.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  A)   Exterior wall of extruder room, on or about January 10, 2018, the open sided edge of the roof by the fixed ladder was not equipped with fall protection, exposing employees to an approximately 17 foot fall hazard.
