29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a) On or about 01/10/2018 - B-Line, Press #33, employees operating oxygen and acetylene to cut off bolts while disassembling a press were doing so with a damaged gauges on a the acetylene tank.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about 01/10/2018 - B-line, press #33, an employee working from an aerial lift disassembling an injection molding machine was working beneath a mezzanine without wearing a hard hat. b) On or about 01/10/2018 - B-line, press #33, employees working beneath an aerial lift manned by another employee were not wearing hard hats while the employee working in the aerial lift handled large tooling and components on an injection molding machine.
