29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. On or about January 8, 2018, employees performing siding installation on a single family residential property from a pole jack scaffold were exposed to a fall of 15 feet when the employer failed to ensure a guardrail system was installed along the open ends of the platform. No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about January 8, 2018, employees performing siding installation on a residential property from a pole jack scaffold were exposed to a fall of 15 feet when the employer failed to ensure the side rails of the ladder used for access extended at least 3 feet (.9 m) above the upper landing. No abatement certification or documentation is required for this item.
