Employees were not wearing safety glasses and were exposed to eye injuries.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: On or about December 28, 2017 at the location 425 Anderson Lane, Forney, TX, employees were using 32-feet, 28-feet and 16-feet extension ladders that did not extend 3-feeet above the landing of the roof and exposing employees to a fall hazard. (a) At least one (1) 32-feet Werner extension ladder was located at the front, east side of the house. (b) At least one (1) 28-feet Werner extension ladder was located on the side, south side of the house. ( c) At least one (1) 16-feet Werner extension ladder was located at the front, east side of the house.
