(a) Orma Molding Press Station #2: The energy control procedure for the molding press did not account for the gravitational energy presented by the pneumatic cylinder.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown was not utilized to avoid any additional or increased hazards to employees as a result of the equipment stoppage. (a) Orma Molding Press Station #2: On or about January 09, 2018, the molding press was not denergized using the procedures established for the molding press prior to an employee conducting repairs on a pneumatic cylinder. This exposed the employee(s) to struck-by and crushing hazards, and amputation injury.
