Worksite: The employer did not ensure that the employee who was exposed to overhead hazards while working in close proximity of the hydraulic arm (arm) of the concrete pumping truck was wearing appropriate type of protective helmet.      On or about 01/11/2018, the arm physically broke and the lower portion of the arm hit the employee's left side of the head. The employee sustained serious head injuries, including ( but not limited to) head trauma and a severe laceration.
