29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use: (a) Nicholas Construction at 3200 South Federal Blvd Denver, CO 80236: On and before January 10, 2018, the employer did not maintain a safety data sheet for each hazardous chemical and lubricant used on the jobsite. The employer used various chemicals at the job site such as, but not limited to, Liquid Nail, Portland Cement, and gasoline. This condition exposed employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards. 29 CFR 1926.59: Note: The requirements applicable to construction work under this section are identical to those set forth at 1910.1200.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Nicholas Construction at 3200 South Federal Blvd, Denver, CO 80236: On or before January 10, 2018, the employer did not provide effective information and training to employees about hazardous chemicals used in the work area, and about the specific information provided in the safety data sheets. Employees used various chemicals and lubricants when working on the jobsite, such as, but not limited to, Liquid Nail, Portland Cement, and gasoline. This condition exposed employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards. 29 CFR 1926.59: Note: The requirements applicable to construction work under this section are identical to those set forth at 1910.1200.

Employees were performing siding work on a ladder jack scaffold platform that was 11' high, and no personal fall arrest system was in use. This condition resulted exposure to a fall hazard of 11 feet.

The employer used two ladders to support a ladder jack scaffold platform, and the ladders were not tied to prevent slippage, and the anti-slip shoes were not in use. Employees were performing siding work on a ladder jack scaffold platform that was 11' high, and no personal fall arrest system was in use. This condition resulted in exposure to a fall hazard of 11 feet.
